ORIGINAL RESEARCH
The association analysis of polymorphic variants common for miRNA-146a, miRNA-218, miRNA-410, miRNA-503 target genes was performed in patients with hemorrhagic fever infected with PUUV. It has been revealed that the genotype rs1127327*AA of the CCDC6 gene is associated with a reduced risk of severe disease form. Further studies of the network of genes that are targets of various miRNAs are needed to elucidate the molecular mechanisms that can influence the onset and development of HFRS.
The article presents the frequency of polymorphic variants of the PNPLA3 gene (rs2294918 and rs738409) in Yakuts (n=150) living in the Republic of Sakha (Yakutia).
In the studied sample of Yakuts, two diplotypes [GG] [GG] and [CG] [GG] are more common. These diplotypes carry the mutant allele G (rs738409) and do not carry the A allele (rs2294918), which has weakening effect on 148M, which in turn promotes the accumulation of triglycerides in hepatocytes. The data obtained on frequencies of the markers rs738409 and rs2294918 of the PNPLA3 gene can be used in the diagnosis of susceptibility to non-alcoholic fatty liver disease (NAFLD) and non-alcoholic steatohepatitis (NASH) to study the genetic mechanisms of human adaptation to cold, as well as the formation of risk groups for these diseases.
Differences in mtDNA copy-number are caused by a complex effect of various environmental factors including chemical, biological or physiological stress, age, and embryonic development. However, to date it remains unknown, which factors and at what age have the most significant impact on mtDNA copy-number and cause greater allostatic load. Therefore, the present study aims to assess the involvement of various socio-demographic parameters in manifesting mtDNA copy-number in healthy individuals aged 18-25 years (N=1065). Within the framework of this study the effects of childhood maltreatment and age on individual differences in mitochondrial DNA copy number were revealed.
The aim of this study is to develop a method for diagnosing PiZ mutation associated with alpha-1 antitrypsin deficiency using real-time PCR technology.
A simple method is proposed for detecting one of the most frequent mutations of the Pi gene PiZ associated with alpha-1 antitrypsin deficiency. Priority exclusion of the PiZ mutation as the most significant one may provide rapid diagnosis and make it more accessible to practical healthcare.
The article presents the results of a study of the bioelectrical activity of the neocortex under conditions of oral administration of an aqueous extract of the herb Lungwort ( Pulmonaria obscura Dum.) to female rats of the WAG/Rij line. The results of the study showed a statistically significant decrease in the power of the theta and beta rhythms of high frequencies by the end of the first week of the experiment in the anterior region of the brain, as well as the delta and theta rhythms in the posterior lobe of the brain. Thus, changes in the theta and beta rhythms of high frequency in the frontal lobe of the neocortex of female rats may be associated with a decrease in excitability, anxiety and fear. The data of our work demonstrated the stimulating effect of Pulmonaria obscura Dum. on the brain of female rats of the WAG /Rij line, manifested by activity on the electroencephalogram.
DIAGNOSTIC AND TREATMENT METHODS
On the basis of the Amur Regional Psychoneurological Dispensary and the Amur Medical Academy, a controlled, open, randomized research was conducted to study cognitive functions in 20 patients with histories of traumatic brain injury in comparison with practically healthy volunteers (n=20).
The results of the study showed that in patients in the late period of traumatic brain injury, the presence of cognitive disorders is recorded in 100% of cases. Mild and moderate cognitive impairment in patients with traumatic brain injury up to 6 years is expressed by a 3-fold decrease in work efficiency relative to healthy volunteers, mental stability, short-term and long-term memory.
Surgical correction of severe idiopathic scoliosis is considered a risky procedure. In many cases, in order to treat stable deformities above 550, a posterior osteotomy such as pedicular osteotomy, resection of the vertebral body, is required. But these procedures cause high neurological failure and bleeding risks, and according to some authors, complications can reach 80% not only during surgery, but also after 6 months surgery.
The technique of arc rotation developed by us in order to correct severe rigid scoliosis makes it possible to significantly achieve correction and avoid postoperative complications associated with osteotomies and discectomies performed in the classic version of surgical treatment.
The article presents a continuous multicenter retrospective cohort study of patients with Behcet's disease (BD). BD is a rare systemic vasculitis with a multi-organ lesion. Given the multi-organ nature of the lesion, BD requires thorough study and attention from doctors of different specialties.
HEALTHY LIFESTYLE. PREVENTION
A review of the literature on the etiological and pathogenetic mechanisms of the development of periodontitis was carried out. The main known etiological and pathogenetic mechanisms for the development of periodontitis require further research in order to expand theoretical concepts that will improve the treatment and prevention measures and improve the quality of life of patients.
ORGANIZATION OF HEALTH, MEDICAL SCIENCE AND EDUCATION
The study provides an analysis of changes in the performance indicators of the state medical organization after the implementation of an internal performance appraisal system of employees in medical structural units. The implementation of this system showed statistically significant positive dynamics of the performance indicators of the medical organization, indicators of personnel and patient satisfaction.
The mortality rates of the population in the Amur region, in the Far Eastern Federal District and in the Russian Federation were studied and the higher rates were found in the Amur region for all years of analysis (2016-2020); the difference in indicators for administrative units is 2.3 times. For the first time, the incidence in the Amur Region is higher than in the Far Eastern Federal District and the Russian Federation as well as diseases of the digestive system are 3 times higher than in the Russian Federation, which determines the need for an in-depth study of the causes of this incidence in the region. The incidence rate of COVID-19 in the region is close to that in the Russian Federation and the Far Eastern Federal District. The higher rates of newly diagnosed morbidity in children and adolescents from the whole population require intensification of preventive work with this age group.
HYGIENE, SANITATION, EPIDEMIOLOGY AND MEDICAL ECOLOGY
The influence of polypeptide vessel complex (PVC) for stimulating the respiratory and cardiovascular systems in a model of experimental hypothermia has been studied. The obtained results indicate that after PVC application the heart rate of rats during cooling was significantly higher and the respiratory rate gets lower as compared with the control. The temperature threshold for respiratory failure after PVC was reduced, and the cooling time of rats to respiratory arrest increased twice as compared with the control. It is concluded that PVC slows down the process of cold respiratory depression and cardiac activity.
The article presents a variant of the grouping of the Republic Sakha (Yakutia) districts according to medical and demographic indicators of children and adolescents health. It is shown that unfavorable indicators are observed in most of arctic districts. In these conditions, it becomes obvious that modern technologies must be widely used to ensure the availability and quality of medical care in settlements with a small population and low transport accessibility.
The aim of the work was to identify the viral etiology of acute respiratory diseases in patients hospitalized in infectious hospitals of Yakutsk. As a result of the study at least one of the tested viruses was positive for 55.6% of the samples. Respiratory syncytic virus; rhinoviruses; methapneumocvirus; parainfluenza I-IV viruses; seasonal coronaviruses; adenoviruses of groups B, C and E; Bokavirus, as well as A and B influenza viruses. The results of the study are necessary to improve and optimize diagnostic tactics, control and prevention of respiratory viral infections.
TOPICAL ISSUE
In order to identify predictors of the development of arterial hypertension (AH) in young people with high normal blood pressure (HNBP), risk factors, hemodynamic parameters, and structural and functional parameters of target organs were studied. A complex of factors that increase the risk of developing hypertension in young people with HNBP has been identified. Among them there is a number of risk factors (dyslipidemia, smoking experience, body mass index and waist circumference, age), intima-media thickness of brachiocephalic vessels, hemodynamic and sizes of the left atrium.
Patients with oncological diseases of various localization were examined. It has been established that immune responses in malignant neoplasms differ depending on the stage of the disease. At the initial stages (I-II), a cytokine reaction is noted, the antibody formation with IgE against the background of inhibition of phagocytic activity, which is associated with the reorientation of the phagocyte to exocytosis. At III-IV stages of cancer, the activity of cytokine and reagin reactions increases in parallel with autosensitization. This period of the disease is characterized by an increase in the phagocytic activity of neutrophils.
Clinical and epidemiological characteristics of coronavirus infection in hospitalized children were studied. The largest number of cases of coronavirus infection was registered among high schoolers, mainly boys. Children infected with SARS-CoV-2 have minimal tourist activity, low vaccination coverage against seasonal influenza; a variety of clinical manifestations is expressed in the moderate form of the disease.
According to some data people who have recovered from COVID-19 often have undesirable consequences of a new disease - post-covid syndrome. We conducted a study of the health status of people who had a coronavirus infection.
Our results showed that after 8 months, people who recovered from COVID-19 complain about the symptoms of post-COVID syndrome. At the same time, the most common clinical manifestations were disturbance (50.9%), arterial hypertension (47.7%), weakness (33.5%), dizziness (32.9%), shortness of breath (27.7%), headaches (24.5%), disorders of the gastrointestinal tract (diarrhea, abdominal pain) (18.7%). Our statistical analysis showed a significant dependence of the increase in the frequency of clinical manifestations: high blood pressure (it was 1.7 times higher), depending on the degree of lung damage.
A study of the respiratory function was conducted in patients who recovered from COVID-19 from 3 to 12 months ago. The appearance of symptoms and disturbances of the respiratory function in the post-covid period did not have significant differences in hospitalized and outpatients, and were not associated with the severity of lung damage in the acute period of the disease, with the post-COVID period, smoking and the presence of respiratory chronic diseases. Obstructive and restrictive types of respiratory disorders were more often observed in female outpatients with post-traumatic disorders, that require rehabilitation measures in order to eliminate multiple organ complications.
ARCTIC MEDICINE
The article presents the dynamics of the key medical and demographic indicators and features of mortality in the Arctic zone of the Republic of Sakha (Yakutia). For the period 2000-2019 in this zone, there is a tendency to reduce overall mortality, including from diseases of the circulatory system and external causes, against the background of continuing birth rates. The mortality rate from external causes was 1.9 times higher than the average for the Russian Federation, and 1.6 times higher for the Republic of Sakha (Yakutia). The infant mortality rate in the Arctic zone was also higher than the average for the republic and the Russian Federation as a whole. High rates of infant mortality and mortality from external causes in the Arctic zone testify to the unfavorable socio-economic situation in the region.
The aim of the work is to analyze the changes in the characteristics of general and local immune responses in children of different age groups. It has been established that in all age groups there is high insufficiency of local and general immune defense, manifested in the decreased activity of phagocytosis, sorption capacity of the epithelium, increase in CEC levels and a decrease in the number of sIgA. In all groups, increased levels of pathogenic and conditionally pathogenic microflora are observed.
On the basis of age-standardized and calculated per 100,000 population cervical cancer (CC) incidence and mortality rates, an analysis of the prevailing over 5 years, from 2016 to 2020, is presented. The analysis of CC data was conducted among nine state entities of Russia, whose territories are wholly or partially located in the Arctic. The calculations carried out using the Friedman’s and Wilcoxon tests, as well as the T-test showed that in these territories there is an unfavorable situation in relation to CC. In most state entities, the CC incidence and its dynamics, as well as the dynamics of CC mortality over five years from 2016 to 2020 show worse results than in Russia as a whole. Nowadays the population of Krasnoyarski Krai, Nenetski and Chukotski Autonomous Okrugs are mostly exposed to cervical cancer. Therefore, these territories especially require the use of new and strengthening of existing measures aimed at the CC prevention, early detection and improvement of treatment.
A case of verification of active brown adipose tissue in a 40-year-old patient, a professional hunter resident of the Arctic ulus of Yakutia, is described. The real brown adipose tissue was detected in samples of adipose tissue from the paranephral fiber. Adipose tissue samples showed high immunoreactivity to the activity marker of this tissue - mitochondrial protein UCP1.
SCIENTIFIC REVIEWS AND LECTURES
The review summarizes the results of studies on applying the principle of neuroimaging for the early diagnosis of Parkinson's disease (PD). The term "neuroimaging" combines various methods of direct or indirect displaying the structure, functions and biochemical characteristics of the nervous system. In-dication of specific defects in brain structures has become possible due to such methods as positron emission tomography, single photon emission computed tomography, transcranial sonography in B-mode, MRI of the brain in SWI mode, and neuromelanin-sensitive MRI (NM-MRI). The ocular component of neuroimaging in PD may be associated with the study of the structural and functional state of various neuroelements of the eye, in particular, with the as-sessment of the state of the corneal nerve fibers (CNF). The difficulties of con-ducting research in this area of neuroimaging may be associated not only with the reliable detection of CNF changes in PD, but also with the assessment of the specificity of these changes, taking into account their potential polyetiolo-gy.
Data on possible ways of influence of SARS-CoV-2 and COVID-19 virus on female reproductive system and already recorded negative consequences are presented in the literature review. The recommendations on pregnancy planning and features of the use of hormonal contraception methods are outlined as well as approaches to specific prevention of new coronavirus infection from the point of view of safety and preservation of reproductive health of women during the COVID-19 pandemic.

The analysis of scientific articles on isolated spinal cord injuries in children was conducted in the literature review (SCIWORA syndrome). This syndrome is mainly noted in children under 8 years, most of the victims being boys. The cervical spinal cord is affected more frequently. The problem of SCIWORA syndrome remains relevant. It is necessary to develop common approaches and standards in the tactics of treatment of children with this severe pathology.
The article presents the macro and micromorphological characteristics of the placenta and umbilical cord during physiological pregnancy and during COVID-19 according to local and foreign literature. A search was made for foreign and local scientific publications on the morphological assessment of the placentas of healthy pregnant women and women with a confirmed COVID-19 diagnosis (positive PCR analysis for the SARSCoV- 2 virus)
The review examines the current aspects of improving the effectiveness and safety of local anesthesia in dentistry, since specialized care is the most common among the population due to the high prevalence of dental caries, its complications and periodontal diseases. At the same time, a significant proportion of clinical cases require anesthesia, where adequate anesthesia is achieved taking into account the psychoemotional state, comorbidity, age of patients, types of anesthetics and trigeminal nerve ramus block techniques. Currently, this problem has not been completely solved. Therefore, there are the researches increasing the anesthetic effect and safety of local anesthesia.
The review presents data on the immunomodulatory role of angiogenic factors and myeloid suppressor cells. The mechanisms that play a key role in shaping the balance of proangiogenic and antiangiogenic factors, the role of endothelial growth factor (VEGF), placental growth factor (PlGF) and myeloid suppressor cells (MDSC, MS) in the development of gestational immunosuppression are shown. Data are presented on the molecular mechanisms of immunosuppression, the expression of check-point molecules that play a major role in the suppression of cellular immunity reactions. The role of tyrosine kinase receptors for proteins of the VEGF family - VEGF-1 (Flt-1), VEGFR-2 (KDR/Flk-1) in the regulation of immune responses has been characterized. Data are presented on the cross-regulatory interaction of angiogenic factors and myeloid suppressor cells and the immunomodulatory effect on cellular immunity responses. Disturbance of these mechanisms may be associated with the development of gestational complications, in particular preeclampsia. Based on the presented data, the possibility of evaluating VEGF and MS in pregnant women as prognostic biomarkers of preeclampsia is considered.
POINT OF VIEW
The analysis of clinical cases of various nosological forms of dementia in patients with neurodegenerative diseases was carried out. The average overall score on the MoCa scale corresponds to severe cognitive impairment. The difference in the age of manifestation in different ethnic groups of patients was established, as well as the clinical and psychopathological features of patients with various nosological forms of dementia, taking into account ethnicity, as well as the features of corrective therapy, were identified. Of the non-cognitive neuropsychiatric disorders, the symptom of agitation and aggression was most often detected.
CLINICAL CASE
The article presents a clinical case of graphospasm - focal dystonia, in which the motor function of the dominant hand is disturbed, which manifests itself mainly when writing and small movements of the hand and gradually leads to disability. The clinical features of this disease at the present stage are considered and relevant methods of treatment are given, in particular botulinum therapy, which quickly and safely alleviate the patient's condition, allow achieving sustainable remission, improving the patient's social activity and quality of life.
ANCA-associated vasculitis (AAV) is a group of diseases characterized by chronic immune inflammation of the wall of small vessels, a polymorphic clinical picture with frequent involvement of the lungs and kidneys, and the presence of circulating autoantibodies to the neutrophil cytoplasm (ANCA). The article presents a clinical case of ANCA-associated systemic vasculitis affecting the lungs and kidneys.
The article describes a clinical case of a severe course of the coronavirus infection (COVID-19) in a woman with comorbid pathology from the practice of the infectious disease department of the Yakutsk Republican Clinical Hospital (YRCB). Type 2 diabetes mellitus, obesity, and arterial hypertension in the elderly woman were predictors of the severe course of the new coronavirus infection, which led to the development of ARDS and caused the death of this patient.
The article presents features of the multisystem inflammatory syndrome associated with COVID -19 in a teenager who had a new coronavirus infection in an asymptomatic form using the example of a clinical observation. The peculiarity of the disease expression is the long-term preservation of changes in the cardiovascular system (dilation of the coronary vessels, separation of the sheets of the pericardium).
