EDITORIAL
The article analyzes the main health indicators of children and adolescents of the Republic Sakha (Yakutia) in the period of implementation of major projects in the health field from 2000 to 2014. It is noted that according to official health statistics rate of primary and general morbidity of children and adolescents of Republic Sakha (Yakutia) increased. Presented structure of the general and primary morbidity of children and adolescents can be an effective tool in improving the pediatric service of the Republic Sakha (Yakutia).
The authors presented results of analysis of cases of lung cancer morbidity of the Republic Sakha (Yakutia) population in 1991-2010. Populational, territorial and temporal regularities are found out.
ORIGINAL RESEARCH
The aim of the study was analysis of the effectiveness of targeted antihypertensive treatment (AHT) based and not based on the correction of sympathicotonia in patients suffering from arterial hypertension (AH), with choleric and sanguine temperament, and the activity of the renin-angiotensin-aldosterone system in phlegmatic and melancholic, for example, reduction of total cholesterol, total fraction of lipoproteins of low and very low density, the value of atherogenic index and increase in high density lipoprotein and magnesium in the blood serum. It is established that by the content of magnesium and atherogenic lipid fractions, and the proportion of persons with complications in terms of AHT, associated and not associated with the correction of psychosomatic status features, highly anxious phlegmatic and melancholic persons are at high risk of developing complications of hypertension.
We determined the threshold level of intranatal augmentation requiring changing labor management. The critical level of intranatal augmentation after abdominal delivery in full cervical dilatation is 82%. 41.7% of newborns with intranatal augmentation 82% and more need intensive care and resuscitation.
We conducted a study to evaluate the hemostatic system parameters in patients with end-stage renal failure, depending on the cause of renal failure and hemodialysis treatment duration. It was revealed that the state of homeostasis in patients with end-stage renal failure characterizes the level of fibrinogen and the activation of the hemostatic markers: soluble fibrin monomer complexes, D-dimers. In patients with end-stage renal failure in the outcome of chronic glomerulonephritis, a great expression of activation of blood coagulation confirm the increased mean concentration of fibrinogen, whereas in the group, which included patients with end-stage renal failure in the outcome of other diseases (type 1 diabetes, type 2 diabetes, systemic disease, polycystic kidney disease, hypertensive nephroangiosclerosis, anomalies of development of kidneys, chronic pyelonephritis, urate nephropathy), such is not different from the norm, and a higher rate of hyperfibrinogenemia, identified in 2/3 patients in this group.
The study included women with pelvic organ prolapse (POP) in the age from 35 to 65 years after hysterectomy by vaginal access in connection with complete and incomplete uterine prolapse and vaginal walls. The greatest expression of tissue degradation we observed in women with POP due to elevated levels of MMP1 and MMP2; they had the lowest TIMP-1 content. It is found that among MMP3 polymorphisms with risk of POP homozygous 5A5A was associated, which increases expression of the gene that has proved its excessive effect on destruction of collagen type I or reduction of biodegradable ability. The low frequency of allele CC MMP2 gene (735C> T) in patients with POP gave reason to consider it as a marker of barrier of tissue biodegradation of the connecting core of the pelvic organs. The mutant allele of CT variant rs 2277698 TIMP-2 gene is more common in women with pelvic floor failure, but without statistically significant differences. Reducing the accumulation of TIMP-1 on the background of the high expression of metalloproteinases details pathobiochemistry of disorders at the pelvic dysfunction.
The aim of this study was to establish organometric features of testicle appendages of immature experimental animals after immunosuppression, simulating the negative impact of exogenous factors on the reproductive system. It was revealed that the testicle appendages of immature animals exhibit an active response to the «soft» immunosuppression. The most active change indicators of organ morphometry are established in the early period after the administration of cyclophosphamide, which is probably due to the imperfection of the structure and functioning of the immature testicle appendages, as well as changes in the immune organs as a constituent of one of the regulatory systems of the body. Leveling of morphometric changes in the later stages of observation can be associated with the normalization of the immune system and the development of adaptation mechanisms in the male reproductive system organs.
METHODS OF DIAGNOSIS AND TREATMENT
The article presents an analysis of the level of compliance to the prevention of thromboembolic complications of patients after large joints replacement in the Republican Hospital №2, Yakutsk from January 2012 to December 2013. Studies have shown a low level of compliance in patients taking parenteral low molecular weight heparins with the transition to indirect anticoagulants.
The authors present a comparative analysis of the efficiency of laparoscopic myomectomy (LM) with bed unit closure with individual sutures and a monofilament self-locking barbed suture. It was found that the use of knotless suture at LM helps to reduce the duration of closure of the myometrium defect, intraoperative blood loss, general surgical risks and the complexity of the intervention, the formation of a wealthy uterine scar due to full recovery of its anatomy. Statistically significant differences in the duration of operations, hospitalization periods, hemoglobin levels and postoperative complications were not observed.
This research demonstrated treatment results of 45 patients with chronic myeloid leukemia who were monitored from 2006 to 2015 year. The article is aimed to evaluate a frequency of achievement of hematologic, cytogenetic, molecular responses in patients administered with tyrosine kinase inhibitors. It is shown that the treatment of choice for CML is tyrosine kinase inhibitor – imatinib. The use of tyrosine kinase inhibitors allows to reach deep cytogenetic and molecular remission in patients with chronic myeloid leukemia which leads to increased survival. Autors noted a high prevalence of primary and secondary resistance to first generation tyrosine kinase inhibitor – imatinib. Investigation of results demonstrated that regular hematological, cytogenetic and molecular monitoring are required for effective disease control.
HEALTHY LIFESTYLE. PROPHYLAXIS
The given article presents the investigation of the adaptation characteristics and the comparative analysis of the adaptive opportunities of the first year students as well as the young men who are recruited in the armed forces. The authors present the conclusion concerning the multidirectional dynamics of the adaptation indicators of the first year students and the young men who do military service during the first year- negative dynamics for the University students and positive one for military servants. Successful adaptation to the conditions of military professional activity is due to more comfort environment for the young men organism.
ARCTIC MEDICINE
The authors have analyzed the features of the RS (Ya) mortality due to external causes, which are on the second place in the structure of causes of death in the Republic. There have been positive trends in reducing the proportion of external causes of mortality in the general structure for the 1990-2015. There were revealed a reduction in the proportion of road traffic deaths and an increase in the contribution of suicide. Considerable disparities by gender still remained. High mortality index of the male on external causes of death is much more important than the total for all causes of death. Excessive mortality from external causes is the main feature of the working age population mortality. In territorial aspect mortality is extremely high from external causes in the northern and arctic regions of the country. A specific feature of mortality due to external causes in Yakutia is a high death rate from accidental drowning; for this reason, there is the biggest gap with those in the Russian Federation and FEFD.
The authors report their study on estimation of informativeness of biochemical tests in men living at high latitudes, for revealing signs of disorders of lipid metabolism. We examined the indigenous and non-indigenous residents of Yakutia in age from 22 to 67 years. It was revealed that significant differences of enzyme activity depended on the degree of adaptation to the conditions of high latitudes. Signs of lipid metabolism have been linked to an indicator of the metabolic state of the body (coefficient de Rytis). Thus, it was found that the ratio of activity of transaminases (coefficient de Rytis) can be used for the formation of a group of persons with signs of lipid exchange and timely preventive measures.
The article dwells on a comprehensive clinical and laboratory study of children with connective tissue dysplasia (CTD), living in the North. The findings suggest that the presence of certain changes in the composition and properties of the oral fluid, which leads to disruption of the structural homogeneity of hard tissue of teeth and reduce their caries. In the surveyed age groups of children biophysical properties of the oral fluid are characterized by a decrease in secretion rate, increased saliva viscosity, the prevalence of type II and III microcrystallisation, decrease of remineralizing potential of oral fluid. In the composition of saliva there were marked decrease in the total protein concentration in children with severe CTD, alkaline phosphatase activity, regardless of severity, concentrations of magnesium and calcium cations, which are essential in saturation by mineral components of tooth enamel hydroxyapatite.
The revealed features of the composition and properties of oral fluid can be considered as specific regional biological risk factors for the formation of pathological processes of organs and tissues of the oral cavity in children with CTD, living in the North.

The article dwells on questions of studies of autologous dermal fibroblasts in the cell therapy for the treatment of skin defects. High clinical efficacy and safety of fibroblasts in the treatment of thermal injuries, non-healing wounds, regeneration of tendons and correction of age-related skin changes are shown.
Literature review is devoted to the actual problem of the use of expert medical systems in abdominal surgery. The article describes in details the existing models of support systems for medical decisions in patients with such common diseases as acute pancreatitis, acute cholecystitis and complicated peptic ulcer disease. The authors made a comparative analysis of modern expert systems and described the basic principles of their construction. The problems of existing systems, analysis of the structure and mechanisms, underlying the design of decision support systems (DSS), examples of various options for building a DSS designed for use in the practice of a surgeon with a description of their key differences, advantages and disadvantages are outlined in article.

The paper presents a clinical case of cystic fibrosis mixed form, severe, combined with cystic fibrosis-related diabetes, a boy of 14 years.
EXPERIENCE EXCHANGE
To assess the degree of hepatic function disorder in patients with widespread phlegmons of the maxillofacial area, complicated by sepsis, the researchers studied blood levels of cytolytic enzymes alanine aminotransferase and aspartate aminotransferase. It is established that at the widespread phlegmons of maxillofacial area complicated by a sepsis the concentration augmentation in blood serum of transaminases is observed, and degree of their augmentation corresponds to the standard criteria of expression of pathological process. Including in a complex of medical actions of intravenous laser radiation of blood in a combination with sodium hypochlorite, especially a combination of ultra-violet radiation of a blood with a double irrigation of wounds by sodium hypochlorite, contributes to normalization of cytolytic enzymes indicators by 6-7 days from treatment initiation.

The article presents the results of research and analysis. According to the results of a comprehensive survey in the visceroptosis patientsin 24.8% of them nephroptosis of varying degreeswas diagnosed. Diagnostic studies included an objective examination, renal ultrasonography, excretory urography and urinalysis data. The features of the nephroptosis diagnosis in visceroptosispatients, the relationship of renal ptosis with omission of the colon are sshown.
A review of the prevalence of renal cell carcinoma and used now classification of the disease is presented. The instrumental methods of diagnosis of renal cell carcinoma and surgical treatment at the present stage of development of medicine are shown. Various methods of surgical interventions and approaches to the choice of a method of surgical treatment of the disease and therefore different results are described. The analysis of results of surgical treatment of renal cell carcinoma based on data published in the current medical literature, including the use of laparoscopic technique is given.
The article presents the clinical case analysis of the patient with diagnosis ulcerative colitis of overall affection with clinical-laboratory results. The patient received baseline therapy with transition to genetically engineered biological therapy with the medicine Infliximab. Due to the development of secondary resistance to the medicine infliximab Golimumab was prescribed. Golimumab was proved effective in the second line of biological therapy.
In order to improve the quality of the diagnostic phase of emergency medical care, improve and enhance the safety of medical and surgical endoscopic technique we examined the possibility of urgent endoscopy in case of insufficient local training of the patient. Based on these results, algorithms and urgent fibrogastroscopy and fibrocolonoscopy we developed ways to improve the safety of medical and surgical intracavitary endoscopic procedures.
