Diagnostic combinations, prodromal period of an acute myocardial infarction among the population of Yakutsk in the age of 25-64 years

Abstract

The purpose of research. Studying within 3 years of diagnostic categories of a myocardial infarction (MI), prodromal period of the diseased.
Material and methods. Research was spent under WHO programs «The Register of an acute myocardial infarction» in Yakutsk among the population at the age of 25-64 years. Since 01.01.2004 till 31.12.2006 in Yakutsk 799 cases of MI are registered.
Results. The diagnostic category “possible MI” is met much more often than “defined MI”. In both diagnostic categories in overwhelming majority of cases the typical clinical picture of disease was observed. But at the same time in a category “defined” myocardial infarction in a significant amount of cases there are also atypical forms of disease. Most often MI develops in house conditions, at the same time MI occurrence was more often on work, in the street - at men or in house conditions - at women. More than at half patients with MI had an arterial hypertension (АH) by strict criteria, in half of cases – angina pectoris (AP), and at every third - old MI. 60, 7 % of male patients are smokers in the present or the past. Distinctions in the characteristic of a painful syndrome during the 2-week period preceding an attack, between groups of the survived and died, are not noted. At the same time more often were met: At died – sudden arrhythmias, a short wind and exacerbation of AP; at the survived - exacerbation of AP and for the first time occurred angina.
The conclusion. Monitoring has shown that within 3 years the MI clinic has not changed and basically is manifested by a typical clinical picture. The fact of presence of considerable number of the diseased having in anamnesis АH, AP, old MI and also intensive smokers that speaks about weakness of preventive work among the population, alerts much.
