Головная боль (ГБ) является одной из самых частых жалоб в клинической практике. Хотя наиболее распространены первичные ГБ (например, головная боль напряжения, мигрень), своевременное выявление вторичных ГБ имеет принципиальное значение, поскольку требует иного подхода к диагностике и лечению. В настоящей статье представлено собственное клиническое наблюдение за пациентом, у которого единственным проявлением болезни Педжета костей являлась ГБ. На основании повышения щелочной фосфатазы в крови было решено провести более глубокое обследование пациента 47 лет с клинической ГБ напряжения. Проведенные МРТ, РКТ головного мозга, остеосцинтиграфия позволили выставить диагноз болезни Педжета костей и назначить патогенетическую терапию.

Headache is one of the most common complaints in clinical practice. While primary headaches (such as tension-type headache and migraine) are the most prevalent, timely identification of secondary headaches is crucial, as they require a different diagnostic and therapeutic approach. This article presents a clinical case of a patient whose only manifestation of Paget’s disease of bone was headache. Based on elevated serum alkaline phosphatase levels, a more thorough examination was performed in a 47-year-old patient initially diagnosed with tension-type headache. MRI, CT of the brain, and bone scintigraphy allowed the diagnosis of Paget’s disease of bone to be established and pathogenetic therapy to be initiated.
