В статье приведены результаты иммунологического исследования больных ювенильным идиопатическим артритом. Было выявлено, что при всех формах заболевания отмечаются существенные изменения цитокинового статуса в сыворотке крови и синовиальной жидкости, при этом степень выраженности зависит от варианта и активности воспалительного процесса. Изучение клеточного иммунитета проводилось с целью дифференцированного подхода к выбору терапии, так как эффективность препаратов различалась в зависимости от варианта заболевания. Наибольшая эффективность при лечении полиартикулярной и олигоартикулярной распространяющийся формы отмечена у ремикейда, комбинированную терапию сандиммуном-неоралом и метотрексатом предпочтительнее использовать при олигоартикулярном персистирующем варианте ювенильного идиопатического артрита.

The results of immunological study of 58 patients with juvenile idiopathic arthritis are given. It was found out that in all forms of the disease substantial changes of cytokine status in serum and synovial fluid were observed, the degree of severity depends on the variant and the activity of the inflammatory process. Study of cellular immunity was held to a differentiated approach to the choice of therapy, as well as the efficiency of the drugs varied depending on the variant of the disease. The greatest efficiency in the treatment of polyarticular and oligoarticular spreading form was observed in remicade, combination therapy with sandimmun-neoral and methotrexate is preferable to use at oligoarticular persistent form of juvenile idiopathic arthritis.
